emarketing: Did you switch to another merchant account or payment provider? Who did you switch to? How are they doing for you?

I've found that even traditional merchant accounts can be tough to deal with for online merchants like us. Unless you have an original card swipe and signature from the cardholder: they are pretty harsh with the merchant when it comes to chargebacks.
