Email Reminder?

I have an insurance related website and I'd like to set up a renewal reminder, whereby people can submit their email and set a date when they want to be reminded by email to renew their insurance.

Any ideas how to do this?

Well according to my point of view, I think that with the help of email reminder you can schedule a number of reminders per event. Reminders are either sent to your email account or to certain people directly.
